Man Dies During Police and Paramedic Response in Waterloo: Critical Incident Investigation Launched

A man has died during a police and ambulance welfare check in inner Sydney, prompting a critical incident investigation. Authorities are now examining the circumstances surrounding the attempted intervention by law enforcement and medical responders.

Just before 2 a.m. on Tuesday, July 15, 2025, emergency services were dispatched to an apartment complex on Pitt Street in Waterloo, New South Wales (NSW), after receiving reports of concern for a man’s well-being. NSW Ambulance paramedics and officers from the New South Wales Police Force (NSW Police) responded jointly to the call.

According to initial reports, efforts were made to sedate the 45-year-old man during the encounter. However, the situation escalated when the man lost consciousness. Despite immediate resuscitation attempts by both paramedics and police at the scene, he was later declared deceased at Royal Prince Alfred Hospital.

Following the incident, a crime scene was established at the unit complex. The State Crime Command’s Homicide Squad, an investigative branch of the NSW Police, has been assigned to lead the critical incident investigation. This protocol is standard when a death occurs during or after police contact.

The investigation will be independently reviewed by the Professional Standards Command (PSC), which oversees police conduct and integrity matters. In addition, the Law Enforcement Conduct Commission (LECC), an oversight body tasked with ensuring police accountability, will monitor the process to ensure transparency.

While full details of the man’s condition, identity, or the events leading up to the emergency call have not yet been made public, the case has already raised concerns about procedures followed during welfare checks. This comes amid growing public scrutiny over the role of police in responding to mental health-related emergencies.

Authorities have not commented further on the specifics, citing the ongoing nature of the investigation.

As inquiries continue, questions remain about how police and paramedics handled the situation and whether protocols were appropriately followed, particularly in high-stress encounters involving vulnerable individuals.
